Getting Your Treatment Records

How do I get my treatment records?

In New Zealand, patients have rights about their health information, and

providers have obligations about records. If a provider has stopped

operating, records may have been transferred, archived, or made available

through a defined process — contact the provider's known successor or the

Health and Disability Commissioner for guidance on your situation.

What can this page establish, and what can't it?

This page gives a practical checklist. It cannot access your records or

guarantee what any specific provider holds.

• Request in writing and keep a copy of the request.
• List what you want: treatment plan, scan files, records, invoices, and
